Most likely, the patient has an SOC asmt and a D/C asmt -- CAREPORTAL always defaults to most current clinical document on file, in this case, the discharge asmt, so that when the Order or Medication was entered, it was entered into the discharge asmt -- to resolve, you simply need to create a new entry with no assessment name selected but set the plan type to 485/POT with a date equal to the SOC asmt date (or recert asmt) -- please edit this '485-only' entry as necessary
if you need to enter orders, medications, etc into an 'older' [not the most current document], you must first select that row in OASIS/POT/Supplemental Order area so that its content is presented and available to you
