Change contact name on claim form

I need to change the information on the UB92's "provider representative" . How do you change it to another  

Go into ADMIN / Clinical Maintenance / AGENCY -- click on the agency name to select / display the form -- you'll see a field in the lower-right labeled 'Contact Person' -- click the down arrow to display a list of workers and simply select desired name from the list -- then click [SAVE]
