Knowledge Base Article #1186:

Care Portal - Assessment won't move to 'validated' status

It is possible that you are clicking the red 'X' in the top-right corner of the validation pop-up instead of clicking the [Close] button.  If so, even though the message in the validation window says that it was successful, the asmt status will remain as 'complete', not 'validated'.
