Problem: Error "ODBC call failed" when reading HSI patient database
Cause: This problem occurs when an RFA 2 (SOC with no further visits) assessment is added in Oasis Gold, followed by a discharge (RFA 9) assessment for the same patient.An RFA 2 is essentially a discharge and no assessments should be entered after that. If they are imported into Synergy this way, Synergy sees the 2 discharges for 1 admission and produces the error.
Resolution: To resolve the problem, one of the assessments will need to be removed.
NOTE: As of GRS204.03, RFA 2 assessments are no longer permitted, so this should not be a problem going forward.
