Knowledge Base Article #723:

M0450 and M0460 are not displaying in type-4 RECRT Assessment

Both M0450 and M0460 are valid questions in the Recert assessment.  Unless you answer YES to the psuedo question (Does this patient have a pressure ulcer?), then you will not see the Pressure Ulcer questions M0450 and M0460.
 
Below is the actual data pulled from CMS' OASIS-Specs for each of the questions.  Please see the line that says "Active on RFA' where it lists the assessment types that contain the question.  Also note at the bottom under the Consistency Required section (*3) where it indicates the question may be either blank or answered for an RFA 4 or 5. 
 
The validation warnings are valid and require no action if the questions are supposed to be blank.
