Steps for admitting a Medicare-certified patient in CarePortal

Question: How do I admit a Medicare-certified patient into CarePortal?

Answer: Here are the steps to admit a patient into Care Portal:

  1. from main CP home page, click CLINICAL then click either ‘New Patient’ or ‘Add New’ link
  2. complete necessary / appropriate fields on DEMOGRAPHICS screen and click [Save]
  3. click ‘Patient Primary Referral’ link; click [New Entry] on next screen
  4. enter referral date/time, primary referral source and projected verbal SOC date; click [Save] then click [Back] to return to DEMOGRAPHICS screen
  5. click ‘Patient Primary Physician’ link; click [New Entry] on next screen
  6. enter primary physician and effective date; click [Save] then click [Back] to return to DEMOGRAPHICS screen
  7. click ‘Patient Profile’ (to display menu options), click ‘Orders’ link; click [New Entry] on next screen
  8. enter discipline; click [Save]; click [New Entry] in Frequency/Duration section
  9. enter frequency / duration based on patient’s staffing needs; click [Save]; click [New Entry] in Scheduler Maker section
  10. enter start / end times, worker and days for schedule; click [Save]
  11. click ‘Patient Profile’ (to display menu options), click ‘OASIS / POT / Suppl Orders’ link; click 'Edit' in grid row on next screen.  Enter assessment date, assessment name, assessor, physician then click [Proceed to Oasis Assessment] if you are ready to enter the assessment, if not, click [Save] and when you are ready to enter the assessment you will need to "edit" and then proceed to Oasis Assessment.
  12. click ‘Home’ link to return to main CP home page, click WORKFLOW then click ‘Confirm Admissions and/or Non-Admit Patients’ link
  13. check box in ‘Confirm Admission’ column, click [Save], click [Continue] then click ‘Home’ link to return to main CP home page
